Demonstration course grade appeal form, whereby a student would appeal their grade based on arithmetic or clerical error, arbitrariness, discrimination, harassment, or personal malice inline with the University's Equal Opportunity/ADA Office's policy summary.

Failing a satisfactory resolution following conversations directly with the chair of the academic department that offers the class, the student may appeal their grade by using this form.
