COVID-19 Food Parcel Order Form and Workflow - Library

Item Description

This form is designed to enable vulnerable residents to select food items they wish to purchase, for them to be delivered to their address and left either at a safe place or doorstep.

The form gives the user an option of either paying online at the time of completing the form or have a call back to take the payment over the phone. For a callback option, we are asking for a code word to prevent any fraud and give the resident confidence in who they are giving their card details to.

The form is integrated with PayBridge payments and Jadu Connect (both are optional) 

This form can be imported into Jadu Central Forms and edited and amended to meet your needs.
